In old age, Mr Meier decides to move to a care home. All information about the treatment he has received in recent years has been added to his health dossier via Health Connect by all the relevant medical service providers. In order to optimise his care, the care home staff can access all this information and constantly update it via Health Connect.

Mr Meier’s recovery is slow. His GP prescribes rehab and sends the payment request to the health insurance company. The rehab provider receives confirmation of coverage from the insurance company via Health Connect. Mr Meier gives the rehab provider the necessary authorisation to access his health dossier.

Since his mobility is still seriously affected by his broken leg and torn ligament, Mr Meier needs help at home. The local Spitex organisation does various household chores for him twice a week. The Spitex employee finds all the important information in Mr Meier’s health dossier.

In order to improve his recovery, the GP has added a medicine to the hospital medication plan. He gives Mr Meier an e-prescription, which is added to the medication plan. The pharmacist checks the e-prescription in the health dossier and gives Mr Meier the medicine. This is noted in the medication plan in the health dossier.

After an operation and five days in hospital, Mr Meier is discharged. His GP will be in charge of his recovery from now on. In the health dossier, his GP can find a complete record of his hospital treatment, a detailed discharge report and the hospital medication plan.

Mr Meier has concussion, a broken leg and a torn ligament. The hospital checks his medical history in order to minimise the risk of incorrect treatment (e.g. allergies). Data relevant to his treatment, such as X-rays, MRI scans, lab results, medication, findings, discharge reports, etc. is added to his health dossier via Health Connect. In future, authorised experts can access this data anytime, anywhere.

During the ski race, Mr Meier suffers a heavy fall. Another skier calls for help. The paramedics who arrive at the scene of the accident soon afterwards call for the rescue helicopter. Mr Meier is flown to hospital. While he is still in the air, the hospital staff can find his emergency contact details in his Evita health dossier.

Mr Meier keeps himself fit. He monitors his sporting activities by wearing a pedometer and pulse monitor on his wrist. He transmits the data it collects to his Evita health dossier via Health Connect. He gives his GP access to his health data and asks him for advice. Mr Meier takes part in a ski race, for which Swisscom Health provides the time measurement and rankings infrastructure. The results are transmitted directly to his health dossier via Health Connect.
